Phase 3
Program & Workforce Design
Teams will design a model that responds directly to local community and school community needs. Rather than introducing a one-size-fits-all approach, this phase centers on thoughtful co-creation, ensuring the model reflects the unique context of each school and local community while remaining durable enough to support long-term success.
Your Phase 3 Supports:
Your SWIM Team will support the tailoring of School Wellness Integration Model to the needs of your local school community.
Our Work Together During Phase 3
Assess Existing Care Models and Student Support Systems
Assist leaders in identifying existing wellness services within frameworks such as MTSS, and considering what should be enhanced/altered to better serve students.
Design a Student-Centered Care Model
Help partners define how educators, mental health and wellness staff, and support staff work collectively to ensure students feel supported, connected, and known.
Support Workforce Strategy (Finding the Right People)
Guide leaders in identifying the roles and expertise necessary to staff the model effectively. This may include licensed clinicians, wellness providers, interns, and other trained professionals whose presence expands the network of protective adults surrounding students.
Clarify Operational Pathways
Support collaborative planning around referral processes, communication structures, and coordination practices that allow partners to work together with clarity and purpose.
Outcomes for Phase 3
A student-centered wellness model that aligns with existing student support systems
Clear roles for the adults responsible for supporting student wellbeing
Operational clarity that supports coordinated implementation
Confidence in the staffing and structures needed to deliver care
“Every community is different. Every community has different needs. It’s very important to understand the particular stressors that may be happening in each individual community.”
