School Wellness
Integration Model
Sustainable school-based mental health powered by a revenue generating model.
Across the country, schools are being asked to respond to rising mental health needs among young people with fewer and fewer resources. The School Wellness Integration Model (SWIM) empowers school systems and healthcare partners to integrate comprehensive mental health services in schools in a financially sustainable way.
About SWIM
The School Wellness Integration Model (SWIM) is a relationship-centered framework designed to help school systems strengthen student wellness through coordinated partnerships with the healthcare system.
Rather than adding another program, SWIM helps build sustainable funding by integrating the health and education systems. The result is a durable funding model for providing lasting mental health supports in schools.
A Six Phase Roadmap for Schools
The SWIM tool provides tailored consultation to leaders, a planning roadmap and implementation resource hub for districts. SWIM is a 6-phase model that provides structured guidance, practical planning tools, and supporting documents that help leaders move intentionally from vision to action and long-term sustainability.
Phase 1
Vision & Coalition Building
Bring together leaders committed to improving student wellbeing and establish shared values through reflection and dialogue.
Phase 2
Partnership and Systems Alignment
Build trusted partnerships between education, healthcare, and community organizations while clarifying roles and structures for coordinated support.
Phase 3
Program and Workforce Design
Design a wellness model tailored to the needs of students and identify the roles and supports required to serve them effectively.
Phase 4
Activation
Launch the model in schools and begin integrating partnerships and supports into daily school life.
Phase 5
Evaluation & Continuous Improvement
Listen to educators, students, and families while reviewing indicators that help refine and strengthen the model.
Phase 6
Building a Movement
Expand the work across schools and embed wellness into the long-term direction of the community.
